
The Rural Health Value team is pleased to share a new profile in innovation, entitled Rural Hospital Participation in Missouri HealthNet’s Transformation of Rural Community Health (ToRCH) Program. This profile describes a new Missouri Medicaid program that integrates healthcare and social services in rural areas. Six rural hospitals are serving as community care hubs, working in partnership with primary care and behavioral health teams and community-based organizations. The profile provides an overview of the program, the intended impact of the program, details about program implementation, funding, and next steps.
